In this role, you will deliver one-on-one physical therapy services in Homeless Shelter to help improve mobility, manage pain, and support independent living. Key Responsibilities Perform physical therapy evaluations and develop individualized care plans Provide physical therapy treatments including strengthening, mobility training, and pain relief techniques in the homeless shelter Educate clients and caregivers on home exercise programs and fall prevention Collaborate with nurses, caregivers, and interdisciplinary team members Accurately complete visit documentation and follow state and federal compliance guidelines Requirements -Active Illinois Physical Therapy License -Graduate of an accredited PT program (DPT or MPT) -Valid driver's license and reliable transportation -Comfortable working independently in homeless shelter -Strong communication and clinical problem-solving skills Preferred Qualifications -1+ year experience in home health or geriatric care -Familiarity with OASIS documentation and EMR systems -Knowledge of Medicaid CCP, DRS, or MCO programs -Bilingual (Spanish/English) a plus Benefits (for Full-Time Roles) -Competitive salary or per-visit pay structure -Health, dental, and vision insurance -Paid time off and holidays -Ongoing training and CEU support -Flexible scheduling Why Join Us?
